How the questionnaire works: Please mark the words that you think your child “understands or understands and says.” There are many words listed on the form and your child may know only a few of them right now. That is perfectly fine! You can mark the bubble if your child says that word on their own and knows something about what it means. “Child-like” pronunciations and special words that you use in your family (e.g., “nana” instead of “grandma”) are okay!

This form was created in the United States, so there may be some words that are particular to the US. If your child uses a New Zealand or other equivalent word (e.g., “pram” or “buggy” instead of “stroller”), you can check the bubbles or simply leave it blank if you prefer.
